After you request an appointment, we’ll contact you via phone or text. Intake forms will be sent through text or the patient portal. Once completed, we’ll offer available appointment times

Patient policies
Rescheduling & Cancellation Policy:
All reschedules require at least 48 business hours notice for new patients and 24 hours notice for returning patients.
Late reschedules may be subject to the same cancellation fees.
Cancellation fees:
New Patients: $350
Returning Patients: $150
Insurance Policy:
We are in-network with Regence, HMA, Blue Cross Blue Shield, and Premera. It is the patient’s responsibility to verify insurance coverage and benefits before the appointment.
We are not in-network with Molina, United Healthcare, Medicaid, Medicare, Kaiser, Aetna, Cigna, Blue Cross Blue Shield plans that originate out of Washington State (including but not limited to Texas, Arizona, California)
Appointment Descriptions
Well Child Exam (45 min) For established patients of all ages. Includes annual physical, sports physical/clearance, exemption forms, and time to address developmental updates, active concerns, or integrative questions.
Well Child Exam – Brief (30 min) For established patients ages 4 years and up with a completely routine check-up and no concerns to discuss.
Sports Physical Only (15 min) For established patients who are current on their well visit and need sports clearance only.
New Patient Exam – Standard (60 min) For new infants or any new patient with active health concerns, developmental questions, or behavioral struggles.
New Patient Exam – Brief (30 min) For children ages 4 years and up establishing care without active concerns.
Return Visit – Brief (15 min) For a focused follow-up on a single issue.
Return Visit – Standard (30 min) For a straightforward sick visit or multi-concern follow-up.
Return Visit – Extended (45 min) For complex or ongoing concerns that need more time.
Return Visit – Virtual (30 min) For established patients who prefer a telehealth visit.
