Dr. Culleton is accepting new patients with Regence, HMA, Blue Cross Blue Shield, and Premera. It is your responsibility to check your insurance benefits if you wish to use them. If Dr. Culleton is not contracted with your insurance you will be required to pay at time of service.
Dr. Culleton’s regular clinical hours are M/T 9am-4pm and Th 10am-2pm. Our office is not an urgent care clinic and does not provide care outside of regular business hours. If you require urgent needs please go to your closest Urgent Care or ER.
New Patients
New patients under 1 month old, please call the office to schedule or reach out to Dr. Culleton directly if you have been placed on her waitlist through a meet-and-greet.
Billing information and health intake forms are required to be completed 24 hours prior to your appointment time through your patient portal. If the forms are not complete, your appointment will be canceled and subject to a $75 fee.
